Here’s how to join our practice:

1. Complete New Patient Forms:
   Download and fill out our New Patient Registration Forms, or complete them in-office at your first visit.

2. Book Your First Appointment:
   Call us at 626-960-2977 Our team will help schedule a visit that works for you.

3. Transfer Medical Records:
   If your child has seen another provider, please complete our Medical Records Release Form so we can obtain prior health records. The form is also included in your registration packet.

4. Insurance and Payment Info:
   We accept most major commercial PPO and HMO plans and offer a concierge-style on-call service for enhanced access and continuity of care. View our financial policy here.
